Quite typically the optimum way to get a deeper voice is through exposure to testosterone. Typically when a male goes through puberty or if a female takes androgenic steroids like testosterone, his or her voice will deepen over about 1 to 2 years. It is much more difficult to lower the voice with surgery. It is sometimes possible to loosen the vocal cords although this is accompanied by a loss of volume typically. Consequently, there are trade-offs in surgery that are not present when hormones can be used.
