A vocal over user who speaks publicly several times per week has developed pain on the left side of his neck for the past month. The pain is from the left vocal process ulceration. He quit smoking cigarettes/tobacco almost 10 years ago.

He has used tobacco and alcohol and declined a biopsy of the lesion so I am not certain of the pathology. The leukoplakia is very exophytic and reminds me of verrucous keratosis. This could represent squamous cell carcinoma as well.

Chronic use of Advair inhaler along with intermittent doses of prednisone lead to a recurrent fungal laryngitis. The white patches grow on the edges of the vocal cords.

51-year-old who has smoked one pack a day for 30 years and consumed significant amount of alcohol is noted a hoarse voice, throat pain and has coughed up some blood. The slightly raised area on the center of the right vocal cord is very stiff on stroboscopy.

There are mucous glands along the inferior vibratory lip of the vocal cords which may become plugged and fill with mucus. This leaves a cyst beneath the surface mucosa of the vocal cord. She has noted the sensation of phlegm affecting her voice for more than 10 years and feels that her voice has become worse over the past year.

Teflon granuloma

After neck surgery 20 years ago, he developed a weak voice from a left sided paralyzed vocal cord. Five months ago Teflon was injected into the left vocal cord for a left paralyzed vocal cord. His voice has gotten worse since the injection.

I had seen her 3 months earlier for her tracheostomy tube which had been removed and she had completely normal vocal cords at that point. The swelling beneath her vocal cords is from the previous tracheostomy tube scar. She then developed hoarseness, which I would attribute to using a Flovent inhaler.

She has used steroid inhalers and develops recurrent fungal infections of the larynx. Presently she is on Pulmicort when the current infection developed. She has almost no voice. The vocal cords are very stiff and during stroboscopy the edges do not align.

Patient with two years of hoarseness. She has sarcoid of her skin for many years which has been treated with steroid injections. She underwent biopsies of her vocal cords which showed non-caseating granulomas and chronic inflammation consistent with sarcoid. Operative photos:

Bacterial laryngitis: This 73-year-old male had intermittent hoarseness in the past. 6 weeks prior to his visit he developed hoarseness that did not go away. On his exam, he had a red, dry, crusty irritation.

Papilloma

A young female who develop hoarseness during pregnancy. The bumpy, red dotted growths on her vocal cord are caused by the virus papilloma. She did not want to have surgery during pregnancy and 4 months later, the papilloma has grown to block off more for airway. She actually can still breathe reasonably well through the small opening.
